Orientation
When there is availability, you will be invited to tour the center and set up a time for parent orientation. At the orientation, the director will review the required childcare paperwork and policy handbook (see below). The teachers will be introduced to the family and the parents will be encouraged to set up a time for a classroom visit with their child. All parents must pay the $25 registration fee prior to having their child attend the program via Procare enrollment link above.

Fees
Initial Non-Refundable Enrollment Fee: $25 per child
Reoccurring Annual State Registration Fee: Approx. $20 per child
Forms
Forms to be completed upon enrollment:
- Child Care Enrollment
- Health History and Emergency Care
- Parent Handbook Acknowledgment Form (updated annually)
- Intake Information for Child Under 2
The following forms must be completed and returned within 30 days after your child’s first day of attendance:
- Immunization Records
- Child Health Report (signed by a physician or nurse practitioner)
*According to State of Wisconsin Licensing Guidelines, enrollment of a child may be terminated if these forms are not completed and returned on time. Parents will receive a 30-day notice when forms need to be updated.
*All forms will be kept confidential and are only available to ACCC staff or a state licenser. Parents can request a copy of these records at any time.
